- Images of you or your home (where appropriate and required for assessment purposes)

- Age, DOB, address, telephone, email and contact details of client and/or family/caregivers

- Sensitive information (information about medical condition, past or present illness, disease or circumstance)

- Where applicable we may need to collect payment details

- Additional personal information that you provide to us, directly or indirectly, through use of our Services, associated applications, associated social media platforms and/or accounts from which you permit us to collect information; and any other personal information requested by us and/or provided by you or a third party.

- For use with third party sub contractors, where necessary for debt collection in case of lawful disagreement with remuneration of services

Your rights and control of personal information

By having read this privacy policy you understand -  If you provide personal information to us, we will collect, hold, use and disclose your personal information in accordance with this Privacy Policy. You do not have to provide personal information to us, however, if you do not, it can affect your use of our Services.

If we receive personal information from a third party, we will protect it as set out in this Privacy Policy. If you are a third party providing personal information about somebody else, you represent and warrant that you have such person’s consent to provide the personal information to us.

Access/changes to personal data

You have a right to access the personal and health information that we hold about you. You can also request amendment any personal/health information should you believe that it is inaccurate.

If we do not agree to change your medical record/personal information in accordance with your request, we will permit you to make a statement of the requested changes and this can be enclosed this with your records.

Should you wish to obtain access/request change to your health record you can ask about our access and correction procedure. Please note that Wild Things Occupational Therapy may recover reasonable costs associated with supplying this information to you.

Website form submissions

Website Form Submissions: when you submit information to this Website, we collect the data requested to track and respond to your enquiries/submissions.
